I had this problem. I traced the cause to be Teamspeak Volume control plugin that mutes the volume when talking. The problem is, the sound never unmutes because the Teamspeak doesn't give control/priority back properly.
To return your sound back to normal.. you need to..
a) Run a Bf3 game.
b) Windows BF, alt tab out of BF3 so can see your windows desktop.
c) Right click system tray speaker icon - select "open volume mixer"
d) Drag the mixer window to make the window larger to reveal the BF3 volume slider.
e) Adjust the BF3 volume back up.
To prevent this from happening again,
Go into Teamspeak Plugins, and uncheck volume control feature.Comment

I loved the plugin that split us after a certain amount of players were in TS (think it was 10). I'm guessing this was removed for crash prevention?Comment
